Open the tab below for the trip you are attending and click the links to download each PDF. Print the PDFs to complete each one, and then scan or photograph the forms to submit as digital files.
Please note where signature fields are included, actual written signatures are required.

STEP 2: Fill out the web form below.
You’ll be able to upload the digital files for each of your completed PDF forms. If you are missing something, you can use the “Save & Continue Later” button. That will email you a unique link that you can use to come back and complete the web form at a later time.
Be aware that First Stamp won’t receive your information until ALL sections of the web form below is submitted AND all PDFs are uploaded. No information will be available to First Stamp until all tasks are complete.
